Fair Paperwork
Paperwork from the Regeneron International Science and Engineering Fair (ISEF) must be uploaded in PDF format into your zFairs account by January 20, 2023. To create a zFairs account, go here. High school students whose project involve human participants, vertebrate animals, or hazardous materials, should complete the ISEF Rules Wizard to determine if they must have their projects reviewed by our Scientific Review Committee before they can begin their projects. The Rochester Regional STEM Fair follows the rules and guidelines of ISEF.
Middle School Paperwork Requirements
All paperwork signatures must be dated before your project start date, as listed on Form 1A, Question 7.

Projects with Human Subjects: A) an informed consent form if needed, and B) a copy of surveys or questionnaires given to subjects, if applicable.
For middle school students whose project involves human participants, vertebrate animals, or hazardous materials, ask your school building coordinator if you need to attend a Scientific Review Committee (SRC) meeting. Most, but not all, middle school projects can be approved by the building coordinator (as Form 1B, section 2 local fair signatory), unless he or she feels a project has complexity that requires a more formal review.
High School Paperwork Requirements
All required paperwork signatures must be dated before your project start date, as listed on Form 1A, Question 7.

If your project involves human participants, vertebrate animals, or hazardous materials, you most likely need to attend a Scientific Review Committee (SRC) review. If applicable to your project, the following paperwork must be dated before the start of your project and uploaded into Zfairs before your review session with the Scientific Review Committee. Bring printed copies of your forms and research plan to your review session.

- Human Participants Form (4), including A) a copy of surveys or questionnaires given to subjects and B) an informed consent form to be used if required by SRC review
- Vertebrate Animal Form: Home, School, Fieldwork (5A)
- Potentially Hazardous Biological Agents Form (6A)
- Human and Animal Tissue Form (6B)
If applicable to your project, the following paperwork requires signatures dated after your experimentation has ended.
